READY Tool: Readiness Evaluation of Transition to Adulthood for Deaf-Blind Youth

Age 18 to 21

Individuals 18 through 21 are now young adults and should be directing their own transition process to the greatest extent possible. Some in this age group may still be in school or receiving educational services. Others will have graduated or exited the educational system. The best practices in this section apply in both cases. They include:

  • Evaluating transportation and assistive technology needs
  • Assessing how the individual performs a variety of tasks in both familiar and unfamiliar environments
  • Continued preparation for all aspects of adult life
  • Cultivating local resources to provide vocational and community-based instruction

Form for recording progress toward best practices ages 18 to 21.
