READY Tool: Readiness Evaluation of Transition to Adulthood for Deaf-Blind Youth

Team Collaboration/Team Adjustments

3.1 Actualize the vision for the future

Ensure that a plan has been created and implemented that provides a good quality of life for the young adult and that the plan can evolve as needs and expectations change. To the greatest extent possible, continue team networking and involvement with the young adult through social media and email.

3.2 Raise awareness

Continue to advocate for and raise awareness of all options, including college, open to young adults with deaf-blindness. Shares examples of customized employment and look for role models to interview and video record.

3.3 Introduce consumer organizations

Introduce the young adult to local, state, and national advocacy groups that support the interests of individuals who are deaf-blind (e.g., American Association of the Deaf-Blind or DeafBlind Citizens in Action).
